Do you have to fit in around your supports instead of your supports fitting in with you? With few supports in Derby sometimes it can't be helped but your Support Coordinator should do their best to find supports that can assist when you need it most.
Transport. If you are being transported, but transferring in and out of the vehicle is made more difficult by the type of vehicle being used, then changes need to be made. Let your Support Coordinator know as soon as possible.
Has doing activities or jobs around the home become more difficult for you but your plan hasn't changed in a while and funding is low? Speak with your Support Coordinator. There are many ways to address this issue and bring about positive changes.
Do you have difficulty living in your house despite all of the Assistive Technology already provided to you? Each case is different but your Support Coordinator can help you understand the process and apply for Specialist Disability Housing. Yes - even if you live in Derby or in a Community.
Are you feeling ignored by your Support Workers, or even bullied? If you aren't comfortable with the people supporting you then let your Support Coordinator know. Your Support Workers should respect you and be professional at all times.
It's important that your supports and your plan are the best they can be. As a Support Coordinator I look through your plan, talk with you and your therapists to fully understand if your plan is right for you.
